Otieno is a former centre-back and defensive midfielder. His career started in 1992 with AFC Leopards where he played until 1994. He then spent three seasons at Tusker FC from 1995 to 1997. He had his best and longest spell in South Africa at Santos FC where he played from 1997 to 2011, making 311 appearances and scoring 34 goals (some sources claim he scored 38 goals). He also played on loan for Cleveland City Stars in 2008. He represented Kenya from 1993 to 2009, earning 105 caps and scoring 9 goals. He was national team captain for 13 years, including during the 2004 Africa Cup of Nations. He's currently the most capped player and possibly longest serving captain in Kenya's history.
Considered one of the greatest captains to ever wear the Kenya national team jersey, Otieno was a towering and prominent player for his leadership and outstanding, distinctive playing style that made him an icon in his country but also in the South African Premier League. After joining Tusker FC in 1995, Otieno matured into a versatile defender and midfield enforcer, known for his commanding presence and powerful long range shots that made him a threat when he joined the attack. Admired for his consistency, professionalism and loyalty, he wasn't a captain only because he filled this role but also for being a motivator and role model with his loud voice on the pitch and tactical awareness that made him the anchor of Kenya's back-line. His trademark long range shots and aerial dominance added that offensive threat that made him a complete package. As he lacked both sprint and acceleration due to his imposing physique, he had to rely on positional sense and game reading ability to engage and then tackle the opposing striker. While he wasn't exactly uncomfortable on the ball, especially during his time at Santos, Otieno wasn't known for being a deep-lying playmaker as his job was to enforce the defensive line whenever he was fielded as defensive midfielder.
