He began his senior career with Everton in 1888, the year the Football League was formed, and quickly established himself as the club's first great left winger. Milward played a vital role in Everton’s 1890–91 league title-winning campaign, scoring 11 goals in 22 matches. Over the course of nine seasons with the club, he amassed 58 goals in 181 league appearances. In 1897, he moved to the short-lived New Brighton Tower before joining Southampton in 1899. With Southampton, Milward won the Southern League title in 1900–01 and helped the club reach the FA Cup Final in 1900, where they finished runners-up. He also earned four England caps between 1891 and 1897, scoring three goals at a time when international fixtures were rare and highly selective. Milward's playing style was defined by his intelligence, decent technical skills, hard shot, exceptional crossfield delivery from wide positions and ability to switch play effectively. Despite being only 5 feet 7 inches tall, Milward was tough and industrious, often outworking larger defenders with his stamina and persistence. Not eager to take the defense duties, he still could manage to drop back when required. He was famous for forming one of the most fearsome left wing pairings with Edgar Chadwick during 1890s.
