His career started in 1977 at Middlesbrough. He was loaned to Australian team Newcastle KB in 1978 and 1982. He eventually spent the rest of his career at Liverpool, where he made 271 appearances from 1981 to 1988, scoring 40 goals. Born in South Africa to Australian parents, he never capped for neither country but played 2 matches for England U21. A fan favourite for his consistency, tirelessness, commitment and high level of fitness, Johnston was a tremendous engine and the kind of player who never stopped running until he dropped. He was the player who more than anyone showed loyalty, a rabid enthusiasm, and a never-say-die attitude in Liverpool team. He enjoyed running and chasing his opponents for the entire match, still going when the other players had already finished their stamina. He troubled the defenders by giving them no time to rest, persistently moving around them, trying to force them to make a mistake or a break off the ball. He was hard to knock off the ball or to mark when he was running due to his erratic style and high technique. Although in possession of courage, strength, pace, unquestionable skills like turns, drag-backs and dribbling, and the ability to whip good crosses over for his strikers, Johnston was at times a frustrating player to watch. He was quite an individualist, which was good from a forward perspective. As a midfielder though it wasn't good that he only chased the ball around on his own instead of adapting himself to the team's needs and playing more with his teammates. Johnston himself would admit that he didn't possess football brain or tactical awareness, and he lacked the adequate technical skill to be a top player. Instead, he just ran around the field like a madman. He was describe by teammate Kenny Dalglish as the enemy of his own self due to his lack of confidence in his abilities that led to psychological drops every time he wasn't in good form.
