Nielsen was a centre forward. He spent most of his career at Bologna from 1961 to 1967, scoring 100 goals in 181 appearances. He played also for Frederikshavn, Inter, Napoli and Sampdoria, where he retired in 1970. Several injuries slowed down his career and Inter bought him in 1967 only to weaken Bologna. He gained 14 caps for Denmark between 1959 and 1960, scoring 15 goals. He unfortunately didn't receive more call-ups due to his transfer to Bologna since Denmark national team didn't accept players who were active in professional leagues.
Nielsen was a very prolific and explosive striker, gifted with uncommon sense of goal, a shot with good power and the ability to score from very difficult positions. Praised for his excellent playstyle, he possessed great agility, the ability to make quick turns, immense speed on short distance but was also insidious with his sprints, moving on the sides, leading the counterattacks and sometimes dribbling the defenders. Capable of resisting most of the defenders attempts to stop him despite not having a huge physique, he was a real in-box striker, very reactive, a decent header, always in position to receive the ball and ready to score after a mistake by the defenders or a goalkeeper save. Lacking of spirit of sacrifice and not prone to collaborating with his teammates, he rarely was the one who deliver a final pass, even though his ball distribution wasn't bad. Although his scoring rate might suggest differently, he was inconsistent, often missing more goals than he scored, sometimes not shining despite scoring two or three times, occasionally playing well only half time, not running enough or being quite stationary.
