Forlivesi was a striker who played as winger, always on the left side. He debuted with Hellas Verona, scoring 20 goals in 31 matches between 1911 and 1914. He then joined Modena where he stayed until his retirement in 1927, playing 118 matches for the and scoring 44 times for the Gialloblu. Forlivesi also won 10 caps for Italy scoring 2 goals between 1920 and 1925. His qualities made him a very efficient player: renowned for his physical characteristics, Forlivesi was described as an athletic and charismatic player with uncommon technique, power and personality who could give rhythm through the entire match; he was very fast, incomparable on the flank and had very good dribbling, often followed by a shot on the run; he could score magnificent goals, sometimes even from outside area; he also had good long passing ability, in fact he was − alongside the right winger − charged to send crosses towards the penalty area and to take corners.
